By typing your full name on this form, I acknowledge that I am signing up for a Mini Portrait Session in support of “Portraits for Vets 2016”, a project to benefit our wounded veterans and their families. A portion of the proceeds will be donated to Wounded Warriors Project. A non-refundable $25 fee is required to hold your spot, and the remaining balance of $110 is due before the photoshoot begins.
